Health Requirements List
Background Check and Drug Screening-Please complete your background and drug screening through Advantage Students. After your results are complete, you will need to share the results with Brenau so they will automatically be shared with us. Results must be completed and shared in order for students to begin classes.
Brenau Health Form-Must be completed by a licensed healthcare professional. You can have this completed at the Brenau Health Center on the Gainesville Campus at no cost to you. Your Brenau Health Form must be completed and uploaded to ACEMAPP in order for students to begin classes.
Photo ID-Please upload a professional headshot or a selfie. You must have a plain background and your photo must be from shoulders up and in professional clothing. Please do not upload your drivers license for this requirement. It will be denied.
Health Insurance-Upload your current health insurance card. If you are planning on getting health insurance through Brenau, please email Ashley McGowan and let her know you will be getting a Brenau health plan.
MMR and Varicella-Please provide proof of two doses OR a titer showing immunity. You do not need a titer if you have proof of two doses.
Professional Liability Insurance-Brenau provides liability insurance for all students. Your Clinical Coordinator will upload that document for you. No action required on your part.
TB-To complete your TB requirement, you must have a skin test, blood test, or chest xray if you had a positive test result. If you have a chest xray, please upload proof of a negative screening and also the TB Signs and Symptoms form as well.
Tdap-Proof of one dose within the past 10 years.
