Clinical Cohort Rotation Requests
- Spring: 2nd Friday of October
- Summer: 2nd Friday of February
- Fall: 1st Friday of June
Preceptorship Requests
- Spring: Last Friday of October
- Summer: Last Friday of February
- Fall: 1st Friday of June
Affiliation Agreement (AA) / MOU Requirement
Your organization must have an up-to-date Affiliation Agreement (AA) / Memorandum of Understanding (MOU) on file before any partnership can be made through ACEMAPP.
- Please contact lauren.farris@eamc.org to verify a current agreement or initiate a new one
- Agreement verification may take up to 2 weeks
- Once fully executed by both parties, a partnership request may be submitted in ACEMAPP
Rotation Request Submission Guidelines
- All fields in the rotation request must be completed in full
- If additional details are needed or an option is not available, include information in the comment section
- A student must be attached to the rotation request
- All student requirements must be completed prior to the start of clinical

Requirements Prior to Clinical Orientation
Students must submit the following:
- Hepatitis B: 3 vaccine series OR positive titer
- MMR: 2 vaccines OR positive titer
- Varicella: 2 vaccines OR positive titer
- Tdap: within the last 10 years
- TB Test: PPD or IGRA within the last 12 months
- COVID vaccination documentation (or declination form, if applicable)
- Influenza vaccine (Oct–March) or declination form
- Current BLS certification (American Heart Association) if required by placement area
- Proof of professional liability insurance (if direct patient care is involved)
- 7-panel drug screen (FDS-7) completed within 30 days
- Suggested option: American Family Care Walk-In Clinics (~$35 same-day 10-panel option)
- Background check completed within 30 days prior to orientation

Additional Notes
- Once the request deadline has passed, unit managers/supervisors will be contacted regarding availability
- Do not submit placeholder requests
- Only verified rotation requests should be submitted
- Misuse of ACEMAPP request submissions may result in denial of future requests
