 Clinical Cohort Rotation Requests o Spring- 2nd Friday of October o Summer- 2nd Friday of February o Fall- 1st Friday of June

 Your organization must have an up-to-date Affiliation Agreement (AA)/Memorandum of Understanding (MOU) on file before any partnership can be made through ACEMAPP. o Please reach out to lauren.farris@eamc.org to verify a current agreement or to initiate an agreement. o Agreements can take up to 2 weeks to be verified. o Once an agreement has been signed by both parties, a partnership request can then be made through ACEMAPP.

 When submitting a rotation request in ACEMAPP, please fill out all information boxes in their entirety. If you have additional information, or could not select what was needed, please add it to the comment box.

 A student must be attached to the rotation request and the student must complete ALL requirements prior to beginning clinical. o Please reach out to lauren.farris@eamc.org or jayme.cook@eamc.org if you have questions about requirements for students

Requirements that must be submitted before Clinical Orientation:  Documentation of all 3 HEP B vaccines OR a positive HEP B titer  Documentation of both MMR vaccines OR a positive MMR titer  Documentation of 2 Varicella vaccines OR a positive Varicella titer  Documentation of recent TDAP received within the past 10 years.  PPD (TB Skin Test) OR IGRA (QuantiFERON Gold or T Spot) done within the past 12 months  COVID Vaccine Card/documentation (or declination form…please request)  Documentation of current Flu vaccine if in flu season (Oct-March) (or declination form…please request)  Current BLS certification issued by American Heart Association if it’s a requirement for the department/area/person you are assigned to  Proof of current Professional Liability Insurance IF clinical involves direct patient care  7-Panel Drug Screen (FDS-7) completed within the past 30 days…American Family Care Walk-In Clinics will do a same day 10 panel drug-screen for $35. This seems to be the easiest and quickest route.  Background check performed no more than 30 days prior to orientation. We highly recommend using www.sentrylink.com

 Once the request deadline is met, we will contact managers/supervisors of the units to see if they can accommodate.

 Do not make a request for holding purposes. Only submit requests that are verified. Misuse of ACEMAPP requests could result in denying future requests.
