Onboarding Prerequisites

Please complete the following onboarding prerequisites prior to your clinical rotation with Legacy Health:

Legacy Health Requirements

1. Students / Faculty: Read Welcome to Legacy Health
2. Students / Faculty: Read Legacy's Married State Preceptorship Model.
3. Students / Faculty: Sign Legacy Nondisclosure Agreement

• Witness signature must be a clinical instructor or school designee.
• Upload signed/witnessed nondisclosure agreement to "LHS Nondisclosure Agreement" requirement in ACEMAPP.

4. Students / Faculty: Read Cosign/Supervised Practice - Request Cosign of Flowsheet Documentation.
5. Students / Faculty: Read Cosign/Supervised Practice - Request Cosign of Note Documentation
6. Cohort & Precepted Students:
   Glucose Meter Training:

• Students do not receive an individual ID for glucose testing.
• Patient blood glucose tests and glucose meter quality checks must be completed with a Legacy-employed nurse in the "Married State".
• Legacy nurse will be responsible for training, observing and testing using their Legacy employee ID number. Testing must be completed in the “Married State” under direct supervision of the Legacy nurse.
  NO exceptions allowed. Two comments need to be added to every patient test completed by a student: “RN notified” and “student nurse testing”.

7. Students / Faculty:
   Complete Learning Modules:

• Contact your school's clinical placement coordinator to receive your login ID and temporary password to Legacy's E+ Learning Management System.
• Password change will be made at mypassword.lhs.org.
• Once you change your password, log in to Legacy’s E+ Learning Management System and complete the assigned self-learning modules.
• How to complete Legacy's mandatory education modulesThese modules must be completed before you begin your clinical rotation:
  • EPIC Nursing Student
  • Non-Employee Mandatory Education
  • POC Nova StatStrip 1.75 Glucose Meter Initial Training and Competency
  • MRI Safety Issues
• Upload training completion certificates to ACEMAPP under the appropriate tab.

8. Students / Faculty: Omnicell Medication Station Registration.

• Login ID and password for Omnicell are the same as the username and password that are used for Epic access.
• Read Omnicell Fingerprint Enrollment.
• Read Cosign/Supervised Practice - Request Cosign of MAR Documentation
• Read Legacy Medications Administration Policy
• No Omnicell / medication access granted to students/faculty assigned to Randall Children's Hospital.

9. Students / Faculty:

• Complete Legacy Student Faculty ID Badge Request Form.
  • Form must be signed by authorized school personnel.
  • Take a fully executed Badge Request Form to any Legacy security office to receive a badge.
    • An entire cohort should NOT be sent to security at the same time; no more than two students at a time should be waiting to obtain their badge.
    • Retain the badge, as you may be assigned to more than one rotation at Legacy.
    • If this occurs, email a picture of the numbers on the back of your badge to dhenry@lhs.org. Deb Henry will forward to Legacy security for remote reactivation of your badge.
• Upload completed form to "LHS-Student/Instructor ID Badge Request" tab in ACEMAPP.

10. Students / Faculty: Read Legacy Care Transfer Communication Process Policy.
11. School Placement Coordinator:

• Complete Legacy access spreadsheet. Legacy Access Spreadsheet.
  • Students and faculty need to be included on this form.
  • Submit at least 30 days prior to the start of the clinical rotation. A** separate spreadsheet is required for each Legacy site.
  • Email spreadsheet to Deb Henry and Legacy Security (email addresses listed on spreadsheet).

• Complete Unity Badge Access self-learning module in Legacy's E+ Learning Management System and upload Unity Badge Access training completion certificate to ACEMAPP under the appropriate tab.
