KP Student Placement Policy: Here
Once assigned to an approved clinical rotation, please complete the following requirements prior to the start of your clinical experience.
Kaiser Westside Medical Center Requirements:
Upload all documents in ACEMAPP through your clinical requirements list.
Education Partners (Schools) Coordinator:
- Submit the Student Access Form at least 3 weeks prior to on-campus orientation date. Include faculty unless they don’t need Epic access or a KP clinical faculty ID badge (i.e., if they will never be on the hospital campus in person).
- IMPORTANT: Please carefully read and review this form. Provide all information requested, including student middle initial and context if the student is or EVER was a KP employee, student, faculty, or contractor, at any KP location. If this information is not accurately reported your student may not have access, access may be denied, or access may be delayed. Email with any questions.
Students & Faculty:
- Review the Student Orientation Course and complete the Orientation Exam in ACEMAPP; this will be available in the assessments section of your profile once you are assigned to an approved clinical rotation.
- Complete the Bloodborne Pathogens Assessment, OSHA Assessment, and HIPAA Assessment, which will be assigned to your profile in ACEMAPP.
- Complete the module Kaiser COVID – 19 101, which will be assigned to your profile once you are attached to a Kaiser rotation.
- Upload evidentiary documents demonstrating all immunization and screening requirements have been met.
- Review the KSMC Parking Map or the KWMC Parking Map (Students and Faculty must park in employee designated spots only, cars may be subject to tow if in member parking)
- Complete the Confidentiality Agreement which is an e-signature form that will be assigned to your profile once you are attached to a Kaiser rotation.
- Review HIPAA Quick Reference
- Review the Student Placement Policy which will be assigned to your profile once you are attached to a Kaiser rotation.
- Review the KPNW Experience Standards which will be assigned to your profile once you are attached to a Kaiser rotation.
- Review Roche glucometer policy which will be assigned to your profile once you are attached to a Kaiser rotation, upload attestation and training attestation after attending on-campus orientation
- Please complete HealthConnect learning modules during on-campus student orientation, and upload attestation of completion within ACEMAPP.
- DEU students only. Complete student profile with the following information and upload to AceMapp AT LEAST 3 weeks prior to on-campus orientation date: 1) Background information: About, interests; 2) Why did you choose nursing?; 3) What are your personal and professional goals?
