Tanner Health Immunization and Credentialing
- Supporting vaccination documents should include verifiable documents (i.e. GRITS or other vaccination record).
- Code of Ethics (Tanner Health) You must complete the Compliance Acknowledgement in ACEMAPP for your rotation at Tanner Health.
- Annual Criminal background check (performed within 12 months prior to rotation)
- Annual Drug screen (performed within 12 months prior to rotation)
- Tanner Health Orientation and Checklist
- Tanner Health User Agreement
Immunization Documentation Accepted
- TB skin test and appropriate follow-up as needed (TB/TST/Quantiferon accepted)
- Hepatitis B vaccination (3 doses) or titers, or declination
- Varicella - documentation of 2 vaccinations, titers, or proof of chickenpox
- Measles, Mumps and Rubella - documentation of 2 MMR or titers
- Tdap vaccination within the last 10 years
- Documentation of Covid Vaccination, if received
- Students/Clinical Faculty must provide proof of Flu vaccination/declination if their clinical experience begins/ends during the current Flu Season (October 1st through April 30th).
Background Checks and Drug Screening
We use InfoMart (Advantage Students) to process background checks and drug screens.
