Faculty Instructor Onboarding Instructions

Please refer to the Faculty Instructor Onboarding Instructions for onboarding. If you are new to Highland, we highly recommend setting up an onboarding meeting with us so we can walk you through the process! Please call (585)341-6709 to schedule a meeting,

The required documents must be sent to Roxanne_Cannarozzo@URMC.Rochester.edu, delivered to the NPD office, or faxed to (585)341-0433 prior to the first clinical day:

1. Annual Achieve/MyPath transcript showing completed Annual Faculty Orientation and Mandatories (initially and then annually)

2. Stryker competency form (only for first-time Highland instructors)

3. The following forms must be completed initially, annually, and if instructing on a new unit:

   • Annual Faculty Checklist (if Highland faculty, please use the Annual Faculty Checklist for HH Employees)

   • Clinical Instructor Orientation form (completed on the unit; not required for HH employees who will be teaching on their own units).

4. The Student Orientation Attestation form must be completed and submitted before or on the first day of clinical

• This form attests that your students have reviewed the Orientation Packet and completed the Post-Test
