2025-2026 Influenza Vaccine Information
Atrium Health Floyd Flu Blitz Schedule
As part of our annual health and safety protocols, all students are required to complete the influenza immunization process by October 31, 2025. This year, we will be using TrackMyVerivax to manage flu vaccine appointments, consent forms, and accommodation requests.
Please follow the steps below to ensure compliance:
Step 1: Create or Access Your TrackMy Account
• If you already have a TrackMy account, click the blue “Access TrackMy Verivax” button. TrackMyVaccines
• If you do not have an account, go to TrackMyVaccines and click “Register”.
• Use registration key: 25SESTUDENT
Step 2: Complete the Survey and Consent
• Navigate to the “Forms and Consents” tile.
• Select Start, choose the applicable option, and complete the survey.
• Acknowledge the VIS document, type your name, and check the Consent to Policy box.
Step 3: Schedule Your Vaccine Appointment
• Go to the “Go-to Appointments” tile.
• Select your preferred location, then choose the Flu Vaccination tile.
• Pick a date and time, confirm your details, and click Finished.
• You’ll receive a confirmation email.
Step 4: (Optional) Submit an Accommodation Request
• If you need an accommodation, go to the “Accommodations and Declinations” tile. Note: If you are applying for a medical accommodation, you should receive a response in 24 to 48 hours. For religious accommodations, the requests is reviewed by a committee and may take up to a week. You will be notified by email or changes to your request so be sure to monitor your email for updates. Once a decision has been reached for a medical or religious accommodation, you will print that document or email with an approval and upload that to the ACEMAPP requirement field.
• Select Request Accommodation, choose the type, answer the questions, and submit.
Step 5: Print Your Vaccine Record
• Go to the “Vaccines” tile.
• Click the + next to “Influenza” and select Download Vaccine Record.
If you need technical support with TrackMy, please submit a support ticket. Responses may take up to 48 business hours and could be routed to your spam folder.
