The following requirements apply to all UH locations. Please note, some locations may have their own specific requirements in addition to these:

Requirements Approved by UH
UH Waiver (electronic submission)

Requirements Approved by School:
Measles, Mumps & Rubella (MMR)
Tetanus, Diphtheria and Pertussis (TDAP)
Varicella (Chicken Pox)
Current Annual Influenza Vaccination
Hepatitis B
Tuberculosis
UH - Background Check
