The school or clinical site may require its members (students, faculty, preceptors, etc.) to answer survey questions via the Custom Fields section of their requirements. This guide will walk members through accessing their Custom Fields questions.

###Step 1: Navigate to your Home Page

To access your homepage, click the hamburger bar drop-down menu at the top right of the blue banner and select "Collaboration."

###Step 2: Access Custom Fields on Home Page On your home page, scroll near the bottom right, and you will see a section labeled Custom Fields, which is located below the Requirements section and above the Learning Materials section.

Custom Fields questions that affect your compliance will have a red "Edit" (Pencil) icon under the Actions column. To complete the Custom Fields questions, click the "Edit" (Pencil) icon, enter the required information, and click the green "Save" button.

NOTE: It may take a while for the system to register your Custom Fields questions as Complete. After you finish submitting your responses, you may click the ACEMAPP logo at the top left of the blue banner to exit the Custom Fields page.

###Step 3: Access Custom Fields on Profile You may also access your Custom Fields questions through your Profile. To access them through your Profile, click the drop-down menu with your name at the top right of the blue banner, and select "Profile."

Under the Navigation panel on the left, click the "Custom" tab to complete your Custom Fields questions.

NOTE: If you have multiple ACEMAPP accounts that need to be merged, and you have already completed your Custom Fields questions, you may be required to complete them again after the account merge.
