Forms Report

The Forms Report allows school coordinators to view the total hours that students have logged for a specific course. This report also shows which forms have been approved and provides the associated rotation ID. You can view the report directly in your browser or export the data as a spreadsheet for your records.


Step 1: Access the Reporting Dashboard

From your Home Page, click the "Reporting" button on the navigation bar to open the Reporting Dashboard.

Step 2: Locate the Forms Report

  • Select the "View All" tab.

  • In the search bar, type "Form."

  • Find "Forms Report" in the list and click "Run Report."

Step 3: Apply Filters

Complete the required filter: Status

  • Current – Students currently assigned to the course.

  • Complete – Students who have completed the course.

  • All – Includes both Current and Complete courses.

Optional filters to narrow your results:

  • Course – Select a specific course.

  • Starting After – Filter by start date.

  • Ending Before – Filter by end date.

After selecting your filters, click "Submit."

Step 4: Export Your Report

To save your results, click the "Export" button in the top-right corner of the report table.

NOTE: Clicking "Generate CSV" will download your report as a CSV file, which can be opened in spreadsheet programs like Excel or Google Sheets or a simple Text Editor.

A rotation is a scheduled clinical experience.
