ACEMAPP Assure: Tips for an Approved Document
Overview
ACEMAPP Assure collects, stores, and reviews requirements, including background check/drug screening results, immunizations, and other requested documents. While the Assure team works behind the scenes, school users have full visibility of Assure member documents, review status, and overall compliance.

Tips for an approved document
- Confirm your full name is on the document
- Confirm that the uploaded documentation matches the requirement name
- Confirm your document is a verifiable and official document - documents must be a printed medical record from a health portal, pharmacy (including receipts), agency, or clinical site
- If handwritten, the document must include either the clinical site name or address. If the document has the provider's signature only, the NPI number must be included.
- Confirm all required fields on school forms are completed
- Ensure any required signatures on the document are complete
- Confirm what you enter as the completion date matches the date on the document
- Confirm your document is legible and is not blurry
NOTE: All uploaded documents must be PDF or image file types (JPEG, PNG, GIF, BMP) and less than 20 MB in size.

Uploading Assure requirements
- Navigate to your Assure home page by clicking your name in the upper right corner. Then, under "Navigation," click "Assure Member."
- Click the red "Manage Requirement" button to view the requirement.
- Read the requirement description carefully to determine the appropriate group and level for your documents. For step-by-step instructions, please see our help desk guide on uploading documentation for tiered requirements: Uploading Tiered Requirements
- Once uploaded, your pending documents will be reviewed and managed by the ACEMAPP Assure team.
NOTE: If all your immunizations are compiled into a single document, you must upload this document to each corresponding Assure Requirement.
