- All hospital and all other clinical facilities' listed modules, forms, assessments, SCORMs, training, etc., must be completed before students enter the clinical facilities.
The clinical Instructor (Professor Praveen) will provide information about student's badges. - Adjunct faculty/instructors in the clinical field will provide information about students' parking information.
All immunization records must be obtained from the local health department or physician's office via the GRITS system. Remember that some minute-clinic, ex CVS or family practice, do not have access to the GRITS. If not on the GRITS record, these immunizations must also be converted to GRITS.
In addition, all out-of-state or out-of-country immunization records/papers must be converted by the health department or physician via the Georgia Registry of Immunization Transactions and Services (GRITS). FLU/COVID/TB records can be on different forms. Also, if you have a blood test, ex., titer or chest x-ray, you must upload additional supporting documentation.
None of these documents cannot be dropped off at any offices on the GTC campus. They must be uploaded to ACEMAPP/ASSURE using your phone or computer via PDF format only.
Note: If any of your documents reflect a different last name than the one on file, please also provide supporting documentation (birth certificate, marriage license, etc.)
Students must have a current CPR to remain complaint, and card must always be current or within 90days of the next semester starting. All students must have a signed statement form from a medical physician/Assistant or a practitioner [MD, Physician Assistant, or FNP] stating they can meet the technical standards and have all up-to-date immunizations or waivers. This form will be located on the ACEMAPP GTC page under "Important Documents." The GTC Health Summary document must have the name of the medical/assistant/practitioner legible, the facility address, and the phone number (stamped or written in). Immunizations:
Hep B Students must have Documentation of at least two HEP B vaccination or proof of a positive titer. HEP B Declination Form which may be obtained from any physician’s office or public health department
MMR Students must have Documentation of two MMR (Measles, Mumps, Rubella) dosed vaccinations or proof of a positive titer.
TDAP Students must have Documentation of a TDAP (Tetanus, Diphtheria, Pertussis) vaccine dated May 2016 or after. Be sure that your Documentation says TDAP (Tetanus/Diphtheria/Pertussis) and not just "NO" TD (Tetanus/Diphtheria). If your Documentation says "TD" it will not be accepted.
VZV-Varicella (Chicken Pox)
Students must have Documentation of two Varicella (Chicken Pox) vaccines or proof of a positive titer.
"NO" History of varicella (Chicken Pox) does not meet the program requirement. As a nursing student you are required to have a yearly TB screening. TB screenings cannot have an expiration date prior to your start date.
FLU/COVID Students will also be expected to receive seasonal Influenza vaccinations and COVID-19 vaccinations.
TB
Students must have Documentation of a recent QuantiFERON-TB Gold or T-Spot blood test.
"NO" TB skin test or PPD will be accepted in any hospital rotation.

Background check and Urine Drug Screen** Students must complete a background check and drug screening to enter any clinical facility. Please visit Step #4-The Getting Started: ACEMAPP/ASSURE/Advantage Students tab for more information.

Handwashing** Some facilities require handwashing and hand hygiene validation by one of our GTC-validated instructors/observers.
When a student is assigned to a rotation that requires handwashing/sanitizer as one of the clinical requirements, the student must review this PPT Orientation slide Click here PPT Orientation . Set an appointment with the lab/clinical instructor to complete successful handwashing.
The instructor will have the form (s) ready for the student during the appointment. After validation, students must upload this form to ACEMAPP in PDF format before any deadline. So, setting an appointment with the instructor ahead of deadlines is essential.
IMPORTANT-When you have been onboarded to ACEMAPP, please go under the tab "All Clinical Requirements."
