Requirements

Documentation of a flu shot administered during the current flu season. Please have your flu vaccine completed by October 1. The renewal date will be set for 10/1 of the following flu season.

Attached is a copy of the information that must be included on your Proof of Immunization for Flu. You may also bring a copy of this document for your provider to complete if necessary.
