General Credentialing Paperwork Information

General Information

1. It is very important that you complete your facilities clinical documentation requirements by the due date. Failure to do so will result in you being dropped from your courses
2. Each facility has slightly different requirements

• Some facilities are members of ACEMAPP and their credentialing is located within the rotation.

• You will be able to view and complete when you are assigned to that rotation.

• Some facilities are not member of ACEMAPP and are considered Entities.

• You will find the credentialing requirements under Directory →Entites and locating the site.

All documents will be uploaded into AceMapp. 3. All paperwork is subject to change, even once you have completed. Facilities are frequently updating their paperwork.

• Please check emails and ACEMAPP for updates each semester.

4. If you are not vaccinated for flu, MMR, Hep B, Tdap and/or Varicella, please let Mrs. Bell know ASAP

• There are some clinical facilties where you cannot be assigned.
• If you are not vaccinated , you WILL be traveling to USA in Mobile, AL for all of your pediatric clinical rotations.
