One of the following is required: Documentation of two MMR vaccinations at least 4 weeks apart; or Laboratory evidence of immunity (titer)

Step One:

Upload Immunization Card from childhood (must show two injections)

  • If Immunization card shows two injections, you are complete
  • If Immunization card shows one injection, have second injection of MMR
  • If you do not have an immunization card from childhood, you must upload two injections of MMR or a positive titer in all 3 components of MMR
