One of the following is required: Documentation of age-appropriate varicella vaccination (1) Preschool age child (2) School age child, adolescent or adult = 2 shots total at least 4 weeks apart; Laboratory evidence of immunity (titer).

Step One: Upload Immunization Card from childhood (must show two injections)

  • If Immunization card shows two injections, you are complete
  • If Immunization card shows one injection, have second injection of Varicella
  • If you do not have an immunization card from childhood, you must upload two injections of varicella or a positive titer.
