HEALTH INSURANCE

Each student must have health insurance while attending the Professional Program. You may be covered through an individual private program or purchase coverage through the Auburn University Student Insurance Plan. Please provide a copy of front and back of insurance card. If student's name is not on the insurance card, proof of insurance is required. Students may upload a 1095 Tax Form or a Declaration Page via Insurance Company

Health Insurance Proof

• Upload Proof of insurance in the form of a 1095 tax document or a declaration page from your insurance company into AceMapp.
